Travel To Switzerland

The passport renewal asks for dates of travel. Am I held to those dates? Can a say June 1 to August 1, but the actual travel is July 9-25? Or does my application have to say the exact dates?

Once I do the renewal and book my flights, what else will I need? Credit Card? What other types of identification? Help!
